Education Forms

When claiming education training or workshop costs with MI, the following forms are required:

Education Incentive Application  MI Sustainable Business Strategy 
Supplier Maintenance Request  Provides customers banking details for MI to refund costs
Agreement to Issue Recipient Created Tax Invoice  Provides permission for MI to generate tax invoice for customer
