When claiming Education Training/Workshop with MI, the following is required;

Education Incentive Application Form (2MB PDF)	MI Sustainable Business Strategy (MI SBS)
Supplier Maintenance Request	Provides customers banking details for MI to refund cost of course/training
Agreement to Issue Recipient Created Tax Invoice	provides permission for MI to generate tax Invoice for customer
Copy of Receipt	Course/workshop that was attended by customer
Once above complete, lodge with MI Education Facilitator (either by email, fax or in person)
